Hills Occupational Therapy (OT) provides professional clinical services, focusing on empowering adults and older individuals to achieve greater wellbeing and independence in their daily lives.

Jacqui's other venture, Equine and Human Connections, has forged a powerful partnership with Memory Nurture. Together, they create unique, horse-assisted experiences designed to support wellbeing, connection, and joy for people living with dementia, neurological conditions, or autism, and their families.

Meaningful Connections
Equine and Human Connections is delighted to partner with Memory Nurture, an organisation dedicated to enhancing the wellbeing of people living with dementia and to promote joint experiences with family members. In addition, Memory Nurture provides support to adults with neurological conditions as well as adults with autism.💚
Together, we’re creating meaningful experiences that foster connection, calm, and joy, bringing people and horses together in a safe, nurturing environment.
Through Memory Nurture’s NDIS-registered Animal Assisted Therapy and aged care support packages, participants can experience the healing presence of horses while sharing special moments with family members.

Occupational Therapy
​Hills Occupational Therapy is owned and operated by Jacqui Self, an experienced Occupational Therapist with over 30 years of clinical practice across the UK, New Zealand, and Australia.
​
Jacqui’s experience spans paediatric through to aged care, offering personalised support to help individuals achieve greater independence and wellbeing in their daily lives.
​
She provides services for both NDIS participants (plan- and self-managed), as well as non-NDIS clients (private health or self-funded), and also works with community aged care clients through home care package providers.
​
All consultations are conducted either within the home environment or, where appropriate, at Equine and Human Connections location.
​
Jacqui is qualified to conduct Functional Capacity Assessments (FCAs), complex home modification assessments, and assistive technology assessments and reports under NDIS funding.
​
Based in the Hills region, Jacqui services areas across the Wheatbelt within approximately a two-hour drive of her base.
